Forma Medical Aesthetics greatly values our patient`s time and their experience. We respectfully request that you review our practice cancellation policy below.

We require a $100 deposit at the time of booking. Due to high demand for our medical providers, last minute cancellations present a challenge as we must allow our patients time for their treatment(s) and recovery.

This $100 deposit will not be applied to the balance of your scheduled services but will serve to secure any future appointments.

Should you at any time discontinue your care with Forma Medical Aesthetics, we refund your $100 deposit.

No-show or appointments canceled with less than a 48-hour notice incur a $100 cancellation fee. Future appointments require a new deposit.

We require a 48-hour cancellation notice provided during regular business hours:

Monday and Friday 9:00 am – 3:00 pm;

Tuesday, Wednesday, and Thursday 9:00 am – 5:00 pm.

We are closed on Saturday and Sunday.

Cancellations via Yelp, Square Scheduler, Facebook, or Instagram are not accepted.
