Here at Forma Medical Aesthetics we greatly value our patient`s time and their experience. We respectfully request that you review our practice cancellation policy below.

A credit card deposit of $100 is required at the time of booking in order to secure your appointment time(s). Our medical providers are in high demand and are scheduled by appointment only. Last minute cancellations are challenging to fill as we must allow our patients to be able to accommodate time for their treatment(s) and recovery.

This $100 deposit will not be applied to the balance of your scheduled services but will continue to hold any future appointments. This $100 deposit is fully refundable should you discontinue your care at any time with Forma Medical Aesthetics. Patients who no-show or cancel appointments with less than a 48 hour cancellation notice will incur a $100 cancellation fee. A new deposit will be required at this time to secure any future appointments.

Cancellations via Yelp, Square Scheduler, Facebook, or Instagram are not considered formal communication of notice and may not be reviewed by staff as these are not secure portals of communication which our practice staff monitors daily.
Consecutive Cancellation Policy: 
-New patients who no call/ no show their appointment will be politely declined to be seen by our staff in the future.
-Established patients with 2 consecutive cancellations within a 4 month time frame will be politely declined to be seen by our staff in the future.
